The paper concerns the problem of physical education of the students diagnosed with health conditions through a facilitating academic physical education and health improvement environment being built up based on the relevant class and off-class process informational support, differentiation, individualization and integration tools. The study offers an application software to help the students design their individual physical education and health improvement routes with a special emphasis on the personally-important values and senses and the individual physical training and health improvement experience building process.
